Parent's Weekend Meal Reservation Form

This system demonstrates the use of ColdFusion in placing an online meal order request for Yale's Parent's Weekend.

There are two parts to this form. The first is sub-divided allowing someone other than the student to order and provide their details as well. If this option is selected all the following relationship fields become required as well.

Step 1: Fill in the following required personal information.
Step 2: Fill in your meal selections. Please note the special offer.
Step 3: Click on the 'Submit Order' button.

Meal Reservation Order Form

Please use our on-line order form to reserve Parent Weekend Meals for Students, Friends and Family. Fill in all the applicable information below and then click the submit button. You will then have an opportunity to review your order before submitting it to the YDS Accounting Office.

Billing Information: All online orders will be charged to the student's Bursar account. If this is not acceptable and you wish to pay by mail via check, please fill in your information and follow the printing instructions on the review page.

Please note: Reservations are non-refundable and will be held at the door.

Form Must Be Received By October 17, 2000. Thereafter, transactions must be made in person and paid in cash at the Dining Services Accounting Offices.
